Report on Your Grant

Grant recipients will be expected to complete the following formal report within four weeks of the project completion date.

  • Attach as a document of no more than four pages.

    Please be sure to include the following details:
    • the number of students, teachers, or people from the community you reached
    • your reflection on the impact of your program. If appropriate, you may also wish to attach sample student work, etc.

          Please mail original receipts documenting how you spent grant money (stapled to a sheet of paper), along with an itemized list of those receipts using the downloadable form here, to the Memorial Library. Receipts should be mailed at the time of completing this report.
        • Please submit all receipts by attaching them to a separate sheet of paper. Receipts should appear in the order in which they are listed below. Mail receipts and this page to The Memorial Library, 58 E. 79th St., #3R, New York, NY 10075
